Hand written evaluations were always a problem for me as a student. My writing is not just unreadable, it's uniquely unreadable. So while the evaluation would make my identity obvious, it might make my feedback worthless.

I beg teachers, professors, and institutions to create digital feedback forms, preferably phone-based. Google forms do this for free. When you do that, I can go somewhere and dictate my responses, and by part of the class, instead of being that SpEd kid who never offers feedback, so faculty never hears their concerns.
